The document has a red circular paper seal. There is a blue ink stamp. The top edge of the third sheet extends over the top edges of the other sheets, and was probably used to bind the leaves together in conjunction with a pair of holes in the upper left corner of each sheet. Purple pencil on docket title notes "176".

The document is laminated with tissue. Edges are dirty. A brown stain is on the lower left corner of the first sheet. Red stains are on the upper left corner, possibly from a wax seal.
