The document has two covered seals and one red circular paper seal. There is a pink multi-lined marginal border on the affidavit. There are also two blue ink stamps. One blue print stamp possibly states "22071". A pink archival ribbon is used to bind the sheets together.

The document is laminated with tissue. The bottom and right edges of the affidavit are worn, dirty, torn and small pieces are missing. The fold lines are dirty and torn. The coloured seas are visible through the paper on the opposite side. A circular brown stain is opposite the paper seal. An Ink smear is present on the lower left corner of the second page.
