The document has one red wax seal, of which two-thirds of it are missing. A loop of green ribbon is fixed to paper under the wax seal. One piece of pink archival ribbon is also visible on the underside of the wax seal. There is also a blue ink stamp.

The document is laminated with tissue. Yellowed, clear adhesive tape covers the overlap of lamination of the first sheet. A brownish stain is present in the upper right corner of page one. The upper left corner is missing on page three. Two holes are in the upper left corner of page three, possibly to bind the sheets together. The paper's edges are dirty, torn and worn.
