The document has one large red wax seal, located in top left corner. Two 2 pieces of green ribbon are adhered to paper under the wax seal. A single piece of pink archival ribbon is visible on page 2, under the wax seal. Purple ink notation are present along the left edge of the document.

The document is laminated with tissue. The lamination is cracked and torn in the centre fold line. Edges are torn, dirty, and worn. A brown stain is present on the lower left corner of page one. A hole and red stain are in the top left corner of page three. The document has ink stains. A red stain is adjacent to docket title area.
