The document is on parchment. Brown ink is used for underlining and a double-lined border. A single red wax seal is present and it is situated on a blue ribbon which is woven through the parchment. A single long piece of ribbon, woven through the folded portion of top edge, is used to hold the sheets together.

The document is brittle. The edges and docket title area are dirty. There are blue stains from a ribbon, and small holes along the fold lines.
