Physical description note:

The document has a single red wax seal, stamped with a bird and the initials "W.M.S". It is located at the end of Simpson's signature.

Note on the state of conservation:

The document is laminated with tissue. The lamination is torn or cracked on the center fold line. Edges are torn, worn and small pieces are missing. There are at least two holes in the top left corner of the appropriate sides indicating they were possibly use for bind the leaves together. There are also two circular cloth reinforcements on page 24.
