Commission of Inquiry into the Air Ontario Crash at Dryden, Ontario (Canada) : The Commission of Inquiry into the Air Ontario Crash at Dryden, Ontario was appointed in 1989 just days after a tragic accident in which twenty-four were killed. The Government of Canada called on Virgil Peter Moshansky to conduct the inquiry. His mandate was to inquire into and report on the contributing factors and causes of the crash of Air Ontario Flight 1363 Fokker F-28 at Dryden, Ontario, on 10 March 1989 (P.C. 532, 29 March 1989). The commission was appointed under Part I of the Inquiries Act. Subsequent Orders in Council relating to this inquiry are as follows: P.C. 679 and P.C. 680, 20 April 1989; P.C. 625, 29 March 1990; P.C. 1187, 20 June 1991; P.C. 1845, 26 September 1991; P.C. 2591, 30 December 1991; and P.C. 668, 2 April 1992.

A separate inquiry under the Canadian Aviation Safety Board had barely got underway when it was halted by the Minister of Transport, who opted for a public inquiry. However, the Board, which was abolished in 1990, gave assistance to the royal commission right up to the time of its dismantlement. The circumstances which led to the crash are as follows. On 10 March 1989, a Fokker F-28 aircraft, owned by Air Ontario, was on a flight from Thunder Bay to Winnipeg with a stop over in Dryden. Flight 1363, as it was known, was cleared for take-off from Dryden on runway number 29 at approximately 12:07 p.m. The aircraft's last transmission was at 12:09 p.m. when the co-pilot radioed that the plane was ready for take-off. Flight 1363 crashed shortly after it was airborne at approximately 12:11 p.m. with a loss of 24 of the 69 persons on board - 65 passengers and a crew of four. The apparent cause of the crash was a build up of ice and snow on the wings of the aircraft.

The scene of the accident was about 950 metres west of the Dryden runway in a heavily wooded area. The aircraft, heavily damaged by the impact, broke into three pieces. It was further damaged by fire. Most of those killed were seated on the left side of the middle section of the air plane in rows one to seven (Commission of Inquiry into the Air Ontario Crash at Dryden, Ontario, Interim Report, 1989).

The report of the commission was published in three volumes consisting of two interim reports and a final report tabled in the House of Commons on 8 December 1989, 11 December 1990 and 26 March 1992, as Sessional Papers 342-4/13, 342-4/13A and 343-4/17.
