Inmate case records are created upon arrival of an inmate at a federal penal institution. They are made up from the inmate admission forms and from other documentation. The inmate is given an identification number by an institution and this number becomes the file number of the individual concerned. The file created is the inmates original record and it follows him from institution to institution. In addition to the inmate's case record, hospital or medical files, psychological and psychiatric records, and the like, are placed on the inmate's case file after their release. In the case of these particular case files, the individuals concerned applied for and received pardons or records suspensions under the terms of the Criminal Records Act.
