Any person applying for permanent residence in Canada, together with their immediate family (spouse and all unmarried children under age 21) must establish by means of a medical examination that they are in good physical and mental health. Such examinations, originally administered by medical officers within the Immigration Department, have, since 1919, been the responsibility of Public Health (1919-1928), Pensions and National Health (1928-1944) and Health and Welfare (1944). According to the Immigration Act, medical officers report their findings to the Canada Employment and Immigration Commission (CEIC) as advice, whereupon the CEIC makes its final decision regarding a potential immigrant's subsequent entry into Canada. Examinations are usually carried out by roster doctors, often connected with Canadian Embassy staff.

Documentation on the files includes a variety of medical documents such as: the medical history sheet (IMS 3), medical certificates (IMS 1), medical notifications to CEIC, the preliminary medical examination form (IMS 1017) which is submitted by the prospective immigrant, laboratory test results, X-ray examination forms (NHW OR 10B), Canada Entry Records (1097), notifications to provincial health services for continued monitoring, and undertaking forms signed by immigrants agreeing to continued monitoring of their health (IMM 535). There is also usually correspondence relating to the procurement or interpretation of test results and various other reports.

The accession consists of an "F sample"; that is, all case files on persons whose surname begins with the letter "F", excluding such prefixes and particles as "de", "von", "y", etc. The records are from the Overseas and National Captial Regions, and cover immigrants from such places as Jamaica, Italy, Hong Kong, the USSR, the Phillipines, and Haiti.
