Physical description note:

The document has two small red circular paper seal, and one large orange circular paper seal. A single pink archival ribbon binds the sheets together in conjunction with the overlap of the bottom sheet enveloping the upper edges of all sheets. Cut corners of bottom sheet along upper edge. Red ink was used for underlining on the docket title.

Note on the state of conservation:

The document is laminated with tissue. A small pink stain is on page one. Sheet seven has dirty and torn edges, and paper is discoloured or of an unusual yellow-green colour.
