The document has one red wax seal. A green ribbon is attached to the wax seal. One pink archival ribbon is used to tie the sheets together. Five green "Registration" stamps are on the document. Two are located on page two, value of $0.10, and three are located on page eight, value of $0.50.

The document is laminated with tissue. A small hole is on pages seven and eight. A brown stain is on the lower left corner of page one, opposite the wax seal. Fold lines are torn.
