The document has one red wax seal with pieces missing. Two pieces of purple ribbon are attached to the wax seal. One pink archival ribbon is used to bind the sheets together. One piece of parchment is folded around upper left corner of each sheet and attached to the wax seal. Three green "Registration" stamps valued for a total of $0.50 are on the document.

The document is laminated with tissue. A brown stain is present on the lower left corner of page one, opposite the wax seal. The edges and fold lines are torn.
