The document has one orange circular paper seal with two pieces missing. Two green ribbons are visible on the underside of the paper seal. A knot, which is loose, of green ribbon (probably should be adhered to seal) is present. Red ink is used for underlining of text and the docket title.

The document is laminated with tissue. There is discolouration of the paper and random smearing of ink. Numerous holes and slashes are in the upper left corner of each sheet, possibly for binding the sheets together. The upper edge of the fourth sheet has a small piece missing.
