The document has red or pink multi-lined border and underlining. One large red circular paper seal is on page three.

The document is laminated with tissue. The red or pink ink has absorbed into paper and ran or smudged, and is visible on the opposite side of the leaf. Edges are dirty and torn. Fold lines are dirty, torn and small pieces are missing. A brown stain is on page two, opposite the paper seal on page three.
