Physical description note:

The document has two red circular paper seals. Purple ink is used for underlining and for a multi-lined purple border. There are also two blue oval stamps.

Note on the state of conservation:

The document is laminated with tissue. The lower edges are dirty. There are holes in the upper left corner, possibly for binding documentation. The fold lines are also dirty and worn.
