The document has a pink multi-lined marginal border. There are also two blue print stamps "00935", and one black print stamp "11321".

The document is laminated with tissue. The edges are torn, worn and the left edge has small pieces missing. Three holes are in the upper left corner, possibly used for binding the documentation. A brown stain is on the upper right edge of page two.
