The document has a pink, multi-lined border. There is also a blue print stamp, possibly "00935", and a black print stamp, possibly "11321".

The document is laminated with tissue. At least two holes, possibly three, are in the upper left corner. The holes were probably used in binding the documentation. The paper is dirty, with brown stains in upper left corner area. The edges are dirty and worn.
