The document has nine and a half red circular paper seals, located by the signatures. There is a purple multi-lined border. A pink archival ribbon binds the sheets together.

The document is laminated with tissue. The paper edges are dirty, torn, worn and small pieces are missing. The top portion of one seal missing. The docket title page is dirty.
