The document has one circular orange paper seal in the lower right corner of the recto of page one, and four circular black and orange paper seals on the verso of page one. A pink archival ribbon is binding the leaves together by means of a pair of holes on each leaf and sheet.

The document is laminated with tissue. The lamination is cracked along the central fold. The paper edges are torn, dirty, and pieces are missing. The fold lines are also dirty, torn and pieces are missing. The ink is faded. A brownish stain is present on upper corner of pages four and five, by the ribbon.
