The document has one large red wax seal, located at the top left corner of page one. Two pieces of green ribbon are adhered to paper under the wax seal. One small piece of pink archival ribbon is also under the wax seal and ribbon. A circular brown stain is visible opposite the wax seal. Purple ink notation is present along the left edge. Two holes in top left corner of page three are possibly to bind documentation.

The document is laminated with tissue. It is cracked along the bottom of the central fold line. Edges are dirty, torn and pieces are missing. A red stain and hole are in the top left corner of page three and through page four.
