The document is on parchment. Sheet one has red ink underlining and a large seal. The seal is covered with an embossed lid on which is stamped "The Seal of the Province of Ontario". Sheets two and three are bound together by a pink archival ribbon.

On Sheet 1, the parchment is dirty along edges, yellow stains are randomly located, and the fold lines are dirty. On sheet 2, the edges and fold lines are dirty, torn and small pieces are missing. A yellowish stain is visible on page four by the holes for the archival ribbon. The purpose of sheets two and three is a mystery unless they were once used to envelope sheet one.
