The document has a single large red wax seal. It is stamped with "Chief Superintendent of Indian Affairs Canada". There is also a single covered seal. A blue ribbon binds the sheets together.

The document is laminated with tissue. It is yellowing, has small holes, tears and is worn along fold the lines. There is brownish staining on the sheets which is possibly from a wax seal. There are also scattered brown stains on pages five and six.
