Physical description note:

The document has a single piece of pink ribbon running through two holes in top left corner. The ribbon was probably used to bind the affidavit and other documentation together.

Note on the state of conservation:

The document is laminated with tissue. Ink smears are in on lower portion. A brown stain is visible, possibly from a wax seal situated on a previously adjacent document. The left edge is torn and pieces are missing.
