Physical description note:

The document pages are two different colours. Sheets one, two and five are light blue, while sheets three, four and six are grey. A double ink-lined border is randomly used throughout. Additionally, two pieces of pink ribbon are used to bind the document through three holes down the left side.

The document is laminated with tissue.
