The document is written in black and red ink. It has a double-lined border on the left side. The top left corner has three holes in it, used to bind the sheets together. There is also a blue ribbon, in four pieces.

The document is laminated with tissue. Small pieces are missing on sheet two, down the center fold line. The edges are torn and worn.
