On 10 March 1989 an Air Ontario F-28 Fokker jet airplane crashed after takeoff from Dryden, Ontario, killing 23 people. Flight attendant Sonia Hartwick was the only one of four crew members on Air Ontario Flight 363 to survive. The federal government appointed the commission of inquiry on 29 March 1989, headed by Justice Virgil Moshansky.
